I hereby grant my permission and consent for the use by The Gatorade Company (hereinafter “Gatorade”) and each of their affiliates, successors, assigns, employees, agents, officers and directors (the “Gatorade Affiliates”), to use my name, picture, voice, image and/or likeness that has been submitted by or on my behalf as part of the award process or that is captured at events or activities in connection with the Gatorade Player of the Year Program (the “Student Likeness”) for the purpose of congratulating the student and identifying the student as a present or past Gatorade Player of the Year nominee and/or winner in any media or format now or later known without further compensation, permission or notification (the “Purpose”).
I release Gatorade and the Gatorade Affiliates from and against any and all claims which I have or may have in the future arising out of Gatorade’s use, adaptation, reproduction, modification, distribution, exhibition of the Student Likeness for the Purpose.
I fully understand neither I, nor any of my family members, heirs, legatees or assigns, will receive any compensation, attribution, fee or other payment from Gatorade or any other party in connection with this consent and release and that, by signing this consent and release, I specifically agree to the usage of my likeness as set forth above and waive any claim for any such compensation, attribution, payment or remuneration in connection with this usage.
I, the parent and/or legal guardian of {athlete_name} do hereby agree, consent to and ratify all of the foregoing on behalf of myself, my minor child and all of our respective heirs and assigns.
I, the parent and/or legal guardian of {athlete_name} do hereby grant permission, on behalf of myself and my minor child, to receive emails from Gatorade.
